The Agreement State program would like to encourage digital submission of applications.  This will ensure the request is processed in a timely manner.

Submissions to NJ DEP Agreement State Program from the Regulated Community:

Licensing

NJRAD Form-313 “Application for Radioactive Materials License”  is used for new license applications, renewal applications and can be used for amendment requests (though not necessary). The link includes the applications and instructions.

Email the completed applications with supporting documents to the Bureau. Please refrain from including the instructions with your application.

Subject line should include the 6- or 7-digit Program Interest number (existing licensees) or “new application” for initial applicants.  This will ensure the request is processed in a timely manner.

Reciprocity

NJRAD Form-241 “REPORT OF PROPOSED ACTIVITIES WITHIN NEW JERSEY JURISDICTIONAL BOUNDARIES” is used to apply for reciprocity with NJ DEP. See the instructions for assistance in filing out the form.

E-mail the completed NJRAD Form 241 to the Bureau. If you have previously filed for reciprocity, include the include the 6- or 7-digit Program Interest number and anticipated work dates in the subject line.

Termination

Terminations of NJ DEP Radioactive Materials Licenses

To request a termination of a specific radioactive materials license, use NJ RAD Form-314 “CERTIFICATE of DISPOSITION of RADIOACTIVE MATERIALS” .

Email the completed form with supporting documents to the Bureau. Include “Termination of NJ Radioactive Materials License” in the subject line with the 6- or 7-digit Program Interest number.

Most terminations are unique and come with their own set of challenges; a reviewer will help you through the process.

Generally though, certain items pertain to all terminations. The annual license fee required by NJAC 7:28-64.4 must be paid prior to any review of a termination request. If all required submissions have been reviewed and approved by NJ DEP staff by December 31st of a given year, licensees are eligible to request a 50% refund of the annual license fee in accordance with N.J.A.C. 7:28-64.4 (f) Annual fee.

Termination requests require a fee which will be either $260, $505 or the full cost of the review depending on the type of license and the activities authorized under it. For licenses authorized for sealed sources only, the fee is $260. All other termination requests require a fee of $505 and may require full cost billing.
