Registration Section

The Registration Section (Section) within the Bureau of X-Ray Compliance (Bureau) is responsible for processing all requests for registration and disposition of x-ray machines. In addition, the Section is responsible for assessing and collecting all fees associated with registered x-ray machines in New Jersey.

The registrant of the x-ray machine must complete and return to the Bureau a “Radiation Producing Machine Registration Form” within 30 days of installation. Complete the form then email or mail it to the Bureau. The Bureau will generate and mail an invoice for the application fee and pro-rated annual fee after processing your registration request.

A registrant must have a radiation safety survey performed for each x-ray installation. The survey must be performed by an individual with appropriate qualifications for the type of the equipment registered. Whenever an registrant sells, relocates, stores or disposes of an x-ray unit, the registrant is required to notify the Bureau in writing within 30 days of such sale, storage, relocation or disposal using the disposition form. Registrants are responsible for all fees until the Bureau receives written notification. Specific instructions for each action are listed below:

To properly dispose of an x-ray unit, contact your local x-ray assembler to assist in these matters. To view a list of Commercial X-ray Assemblers click here. These companies have indicated to the Department that they perform one or more of the following services: sell, repair, purchase and dispose of x-ray units.

Sale

  • Notify the Bureau in writing within 30 days of such sale using the disposition form
  • Include the pertinent machine information including the registration, model and serial numbers.
  • Provide the name and address of the new registrant and/or new location of the machine
  • Indicated the status of “sale/sold” on the disposition form.

Relocation

  • Notify the Bureau in writing within 30 days of such relocation using the disposition form.
  • Include the pertinent machine information including the registration, model and serial numbers.
  • Provide the name and address of the new location of the machine.
  • Indicated the status of “relocated” on the disposition form.
  • Send/Provide the Bureau with an updated registration form with the new location of the machine.
  • Hire a qualified individual to perform a survey of the relocated unit and send copy to the Bureau within 60 days of registration.

Storage

  • Notify the Bureau in writing within 30 days of such sale using the disposition form
  • Include the pertinent machine information including the registration, model and serial numbers.
  • Indicated the status of “stored” on the disposition form.

Disposal

  • Notify the Bureau in writing within 30 days of such sale using the disposition form
  • Include the pertinent machine information including the registration, model and serial numbers.
  • Indicated the status of “disposed” on the disposition form.

Any planned x-ray installation for period not to exceed 60 days is eligible for temporary registration. Temporary registrations are exempt from the annual registration fee; however, a $40.00 application fee is still required. To obtain a temporary registration follow the instructions below.

Email:

  • completed and signed registration form
  • copy of the radiation safety survey of the environs
  • specific machine location information and its intended use

 

Once the survey has been received:

  • the Bureau will register the x-ray machine assigning a temporary status
  • the Bureau will send a validated copy of the registration with a cover letter to the registrant
  • An initial application fee of $40.00 will be invoiced and mailed to the registrant.

 

No temporary registration will be issued until a survey has been received.

An initial application fee of $40.00 is required for each new x-ray machine registration.

The first year registration fee is a prorated amount of the annual fee based on the number of months remaining before the next billing cycle.

Facilities are invoiced annually for all active x-ray units. Invoicing occurs over a 4 month period and all registrants have 60 days to pay their annual registration renewal fees.

  • Facilities whose business name begins with 0-F are invoiced on or about July 1
  • Facilities whose business name begins with G-L are invoiced on or about August 1
  • Facilities whose business name begins with M-R are invoiced on or about September 1
  • Facilities whose business name begins with S-Z are invoiced on or about October 1
