How to apply for a license?

Please submit NJRAD Form-313 (Revised July 14, 2008) along with associated documentation listed on the form.  

How do I move addresses?

Submit an amendment to add the new location. Move to the new location. Submit an amendment to remove the old location, including information listed on NJRAD Form-314 .  

How should I submit an amendment?

Email all amendment requests to the Bureau. Include your Program Interest Number (six or seven-digit number on you license) and the licensee’s name as it appears on your current license.

How to terminate a license?

NJRAD Form-314 Termination (2024-7) should be submitted, along with associated documentation listed on the form. 

How to change RSO?

Please provide contact information for the individual proposed, training documentation, and a delegation of authority in an amendment.  

What information is required to add an Authorized User?

Please refer to the NRC’s Medical Uses Licensee Toolkit to find the Form 313A series that can be submitted to add an authorized user to a particular use on a license. If a user is being added to a 35.1000 use, please refer to the guidance for the specific type of use listed in the NRC’s Emerging Medical Technologies page.

Do I need a license to have personal check sources as a consultant?

Yes, please refer to 10 CFR 35.65.

Do I need to wait until an amendment is issued to start using a new area of use, or before a proposed authorized user starts working?
