Below are reports pertianing machines that can be found at facilities and the annual registration fee for each machine type in the facility discipline.
This report can be used to verify the active machines registered at a facility and the annual registration fee for each machine type. It is beneficial to utilize this list when you receive your annual invoice or if you have any questions regarding the amount due. You will need your facility identification number or PI number on your invoice.
The lists below give you the annual registration fee and machine category for x-ray units. Click on the facility type that aligns with your office for a complete list.
Below are lists to be used by facilities to find individuals who are qualified to perform radiation safety surveys and quality assurance (QA) surveys. Choose the type of survey that the facility needs and the choose from the list. If you need more than one type of survey, such as a QA survey and a safety survey, choose the QA option because most of the medical physicists do perform both.
If you cannot find an individual close to your facility, call other areas because these individuals do travel to other areas in the State. If you are a facility that is located outside of New Jersey and have a radiation physicist who performs your surveys in your state, have them contact NJ to have their credentials reviewed by the Bureau to become qualified in NJ.
