It is beneficial to utilize this list when you receive your annual invoice or if you have any questions regarding the amount due. You will need your facility identification number or PI number on your invoice.<\/p>\n The lists below give you the annual registration fee and machine category for x-ray units. If you need more than one type of survey, such as a QA survey and a safety survey, choose the QA option because most of the medical physicists do perform both.<\/p>\n If you cannot find an individual close to your facility, call other areas because these individuals do travel to other areas in the State.
