How to obtain a replacement renewal application:<\/h3>\n

All license renewal applications are expected to be mailed on October 1st of every even numbered years and should be received no later than October 15th. If it is past this date, and you wish to mail in you renewal application<\/a> to obtain a replacement renewal application.
